How do I register with a Check or Purchase Order?
If you would like to register using a check or purchase order, please print a copy of your online registration form, include a list of all individuals covered by the check or purchase order if registering multiple individuals, and mail it in with your payment to the address below. Are Consumer Scholarships Available? A limited number of scholarships are available. Please click the Scholarship Opportunities link on the menu at the top for more information.
What is a discount code? We use discount codes for registrants that qualify for a discounted registration rate, some examples of this are: Sponsors, presenters and groups. *Only one discount code may be applied per registration and may not be used in conjunction with the student or group discounts*
Are there group discounts? Groups of five or more registrants qualify for a discounted registration rate. To learn more about the group registration discount contact Karen LaPlante at firstname.lastname@example.org

*To qualify for group savings, groups must register together and pay using one PO, Check or Credit Card.
Are there student discounts? Students enrolled with a minimum of 6.0 hours for graduate students, or 12.0 hours for undergraduate students that send in proof of enrollment for the Fall 2015 semester and a copy of their Student ID via email to email@example.com, or faxed to the attention of Elizabeth Edwards at (918)585-1263, can qualify for a 30% discount. Once verification is complete, student participants will be emailed a discount code to use with their online registration.

If I’m a sponsor, do I get free registration? Our sponsorship levels qualify for a discount code. To become a sponsor click the Sponsorship & Exhibitor Opportunties link on the menu at the top for more information.
How many continuing education credits are available for the conference? A full registration qualifies for 14.0 CEUs. For all details on continuing education information, please click the Continuing Education link above.
Are conference registrations transferrable to other individuals? Registrations are transferrable up to two-weeks prior to the event.
What is the cancellation/Refund Policy? A complete refund for conference registration will be made only if written notification is made to Mental Health Association Oklahoma on or before September 3, 2015. There will be a $50 processing fee for all refunds requested after September 1. No refunds will be granted for registration cancellations made after September 11. Registration fees are not transferable to future events.
