Oklahoma’s Healthy Transition’s Initiative

The Oklahoma Healthy Transitions Initiative (OHTI) focuses the best efforts of our state and local-level partnerships on our youth and young adults ages 16-25 with serious mental health conditions and their families.

On average, 200 young people turn 18 while in Department of Human Services custody in Tulsa County. One-out-3 of these young people experiences homelessness, 
50% will be jobless and 95% of the employed will have an earned-income level below the poverty line.

The referral form and release must be read and filled out completely by the referral source, the referred person and the parent/guardian if the person is under 18 years of age. The referred person or the parent/guardian’s initials by the following bullets and signature on the accompanying release must be present for the referral to be considered by the Review Team.
